Gleneagles Hospital Hong Kong launches MoniCare remote care service programme with free trial quota

Striving to raise public awareness of disease prevention and self-management of health, Gleneagles Hospital Hong Kong (Gleneagles) has recently launched a new remote care service programme, MoniCare, offering patients personalised healthcare experience in the comfort of their homes. Gleneagles’ healthcare professionals will support participants to achieve their health goals holistically via providing remote monitoring of participants’ vital signs and regular nurse follow-ups and doctor consultations.   Featuring hypertension management in the initial phase of the programme, Gleneagles is launching free trial service for members of the public to encourage them to actively manage their own health and ensure early intervention of chronic diseases.

 

MoniCare participants can measure blood pressure every day with the smart blood pressure monitor provided by the hospital.  By connecting the device via Bluetooth to My Gleneagles SmartHealth mobile app which is integrated with the AI-based health monitoring function, participants’ health data will be recorded in and synchronised with the app.  Participants will be able to set their personalised health goals with the healthcare team upon joining the service, and nurses will provide ongoing support through regular remote review of relevant data, follow-up calls, progress tracking and providing health advice. The service is complemented by a prescribed number of doctor consultation sessions. Participants will receive immediate app notifications if abnormalities in health data are detected.  They can also seek support through the hotline or making an appointment for consultation with a Gleneagles’ Family Medicine specialist if needed to facilitate early medical intervention.

 

Following the free pilot of the remote care service for 190 eligible community participants and its existing patients, Gleneagles is extending the service to the public and offers a 6-month free trial service for 50 members of the public, on a first-come, first-served basis, subject to a preliminary suitability assessment by nurses.

Case sharing

A patient of Gleneagles was referred to join the MoniCare remote care service programme during which health data from the daily blood pressure readings measured at home was monitored.  The patient had once experienced high blood pressure at 200/130 mmHg.  The healthcare team noted the abnormality, followed up with the patient, and alerted the attending doctor.  With timely detection of the condition and follow-up doctor consultation, the patient's drug dosage was adjusted and his blood pressure level returned to normal, followed by ongoing monitoring by the healthcare team.
